Seizure disorder. Depression. Monitor blood counts, liver function with long-term use. Avoid abrupt cessation. Change dose gradually. Elderly. Debilitated. Suicidal tendencies. Drug or alcohol abuse. Renal, hepatic, or pulmonary dysfunction. Discontinue if paradoxical reactions occurs. Psychosis, pregnancy, nursing mothers: not recommended. Reevaluate periodically. Therapy for >4 months.

Generic Name and Formulations: Lorazepam 0.5mg, 1mg+, 2mg+; tabs; +scored.

Initially 2–3mg daily; range: 1–10mg daily. Give in 2 or 3 divided doses, with largest dose taken at bedtime. Acute narrow-angle glaucoma. Benzodiazepine. Elderly or debilitated: initially 1–2mg daily; adjust gradually if needed. Not recommended.
