Dosing and Administration — For moderate to moderay-severe pain in adults, initial oral dosing is one 7.5/500 tablet of hydrocodone/APAP.

For rapid relief of pain, taking on an empty stomach or with food low in carbohydrates would provide fastest absorption. Taking hydrocodone/APAP with food decreases the absorption rate of the acetaminophen component although this effect appears most significant when taken with food high in carbohydrates. Many patients find it necessary to take hydrocodone/APAP with food or milk to minimize gastric irritation.

In EMS systems without access to narcotics, protocols often call for administration of acetaminophen or other non-narcotic analgesic agents to patients with pain.

“Doctor shopping” to obtain additional prescriptions is common among drug abusers and people suffering from untreated addiction. “Drug seeking” behavior is very common in addicts and drug abusers. Drug-seeking tactics include emergency calls or visits near the end of office hours, refusal to undergo appropriate examination, testing or referral, repeated “loss” of prescriptions, tampering with prescriptions and reluctance to provide prior medical records or contact information for other treating physician(s).

Because many drugs are excreted in human milk and because of the potential for serious adverse reactions in nursing infants from hydrocodone and acetaminophen, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.

Hepatotoxicity - Acetaminophen has been associated with cases of acute liver failure, at times resulting in liver transplant and death. CONTRAINDICATIONS WARNINGS AND PRECAUTIONS ADVERSE REACTIONS.
